The HIE Spotlight Stories showcase the progress made throughout the Commonwealth in the implementation of HIE. Each story describes a challenge, how HIE provided an effective solution, and the impact this had on workflow efficiencies, communication, and patient care. PhysicianOne Urgent Care is a network of 23 urgent care centers throughout CT, MA, and NY. It offers walk-in, appointment-based, and telehealth treatment for non-life threatening illness and injuries, providing an alternative to Emergency Room and primary care visits.

HealthFirst Family Care Center is a Federally Qualified Health Center located in Fall River providing medical and dental care. It serves minority communities and its service area includes Portuguese, Hispanic, Cambodian, African American, and Brazilian Portuguese populations in the region.

Many Home Health Care Agencies seek to collaborate with medical care organizations to implement electronic information flows to enhance in-home patient care.

My Care Family, also known as the Merrimack Valley Accountable Care Organization (MVACO), is a partnership between Greater Lawrence Family Health Center, Lawrence General Hospital, and AllWays Health Partners.

Floating Hospital for Children is a full-service pediatric hospital at Tufts Medical Center. As it has affiliations with many community hospitals, patients can seek care in their own community without going to Boston.

Family Doctors is a North Shore physician practice that offers many services often seen in larger medical offices in a smaller community setting to allow for more individualized care. Circle Health is a collaboration of physicians, hospitals, and other providers and organizations whose goal is to provide complete connected care in their community through innovative programs and improved care management for primary, urgent, and specialty care.

The Children’s Behavior Health Initiative (CBHI) ensures that MassHealth-insured children with significant behavioral, emotional, and mental health needs and their families receive services they need to be successful.

Reliant Medical Group is a group medical practice in the MetroWest and Central Massachusetts area. Reliant focuses on outpatient ambulatory care and partners with and transfers patients to several hospitals in the region for emergency care.

Upham’s Corner Health Center is dedicated to providing comprehensive and personalized care to all patients. Their goal is to offer each patient continuous primary care in comfort and convenience. It offers community-based health and social services.

Brockton Neighborhood Health Center has provided services to its community for over twenty years. The center is dedicated to serving low-income, diverse, medically underserved patients in Brockton and the surrounding areas.

Cape Cod Healthcare provides accessible healthcare services to residents and visitors of Cape Cod. Their mission is to coordinate and deliver accessible health services and high quality care to everyone. CCHC partners with health and human service providers to coordinate care.

The Dimock Center, an urban health center, provides Boston residents with convenient access to comprehensive high-quality, low-cost health care and human services. Dimock handles more than 76,000 visits annually from 17,000 unique patients that represent a wide range of socioeconomic and ethnic backgrounds.
